2. Health Statement - Signed by your child's healthcare provider.
3. Admission Forms - Please ensure both parents' names are included, along with individual email addresses.
Additionally, please attach a recent photo of your child to the admission paperwork. We also ask that you include a brief summary of your child, covering such details as their favorite toys, activities, favorite foods, when they started walking, and their first word.
